Yusuf's Preview — a PDF reader and annotator for iPad and Mac.

Tap + at the top right and choose a file. The app keeps its own copy in your iCloud Drive, so the document opens on every device you sign in to without hunting for the original. Inside a folder, + also lets you move in something you have already added.
Use the folder button beside the app's name to make one. Press and hold a folder in the sidebar to rename or delete it. A document's own menu has Move to, including straight into a new folder.
Tap the pen to start. On iPad this is Apple's own tool palette, so pencils, highlighters and the eraser all behave as they do elsewhere. In Settings you can limit markup to Apple Pencil, so resting a hand on the page never draws.
Saving is deliberate rather than automatic — press Save when you want your markup written into the document. As a safety net, the app also saves if you swipe the app away while there are unsaved changes.
Press and hold anywhere on a page and choose New Chapter Here, or select text and make a chapter from it. Chapters appear in the sidebar under the Chapters tab, in the order they occur in the document. A chapter added between two others takes the ones below it as sub-chapters.
With Handoff turned on, opening a document on your iPad puts the app in your Mac's Dock, and the other way round. It opens at the page you were reading.
Tools in the sidebar merges several PDFs into one, or splits one into parts. Drag to change the merge order; in Split, tap between two pages to cut there and tap a page to see it larger. Both make new documents and leave the originals untouched.
In your own iCloud Drive, in a folder named after the app. You can see it in the Files app on iPad, or in Finder under iCloud Drive on a Mac.
No. Importing makes a copy, and your markup goes into the copy. If you want your annotated version back where the original came from, use Replace Original in the document's menu. To send a copy elsewhere, use Export a Copy.
The document is in your library but has not come down to this device yet. It opens as soon as it arrives. Check you are signed in to iCloud and have a connection.
Give iCloud a moment to sync. If it persists, open the Files app and check the documents are visible in the Yusuf's Preview folder there.
Both devices need to be signed in to the same Apple Account, with Bluetooth, Wi-Fi and Handoff switched on. Handoff is under Settings → General → AirPlay & Continuity on iPad, and System Settings → General → AirDrop & Handoff on a Mac. The icon is also hidden when the receiving device already has that same document open.
Saving is manual. If the Save button is available, there are changes still to write.
The app is built around iCloud so that a document opens on every device. Without it, documents stay on the device you added them to.
